Application Process

Internal or External Review? First, determine whether you are applying for Internal or External review, as the instructions differ. They also differ depending on whether Internal projects are for new or continuation requests.

Continuation or New Request?

Continuation – Renewal of Current, Ongoing Grants
The deadline for submitting a Continuation Request is September 2, 2008.
In lieu of multi-year grant commitments, ESFA offers grantees the option of requesting renewal of current projects through a simple continuation request rather than requiring a full proposal each grant cycle. Continuation requests should be in the form of a brief letter and should include the amount of the request and a simple budget. Briefly list and describe any changes that have occurred in the organization or with the scope of the project since the original proposal (i.e., changes in personnel, address, phone number, or email address; any change to the budget or purpose of the grant; any changes among collaborators). Carefully consider whether your project actually qualifies for continuation or renewal. If there are changes to the scope of the grant, other than minor adjustments, you should seek funds as a new project request. If you are not sure of your project’s eligibility, contact Shirley Hamilton to discuss. The continuation letter must be signed by the primary contact for the request, as well as by an officer or top staff (executive director, president, department chair, dean, etc.) of the requesting organization.

 

New Grants – First Step is the Letter of Intent
The deadline for submitting a Letter of Intent is September 2, 2008.
To request funds from the EyeSight Foundation of Alabama, an organization should first write a one- to two-page Letter of Intent that briefly describes the organization’s mission and activities, the area and population(s) served, how the funds will be used, and the total funds requested.

If requesting core operating support to underwrite the regular, ongoing eye-related activities of the organization, please explain how the funds will support the organization’s infrastructure, such as by providing salaries for key staff, purchasing equipment, and so forth. If requesting project funding, please include project goals, leadership and duration. Your letter will be processed most accurately if you clearly designate the ESFA priority area from the list on page 2 that your request addresses (example: eye care for the medically indigent). No application form is needed, and formal proposals are not accepted at this preliminary stage. Those encouraged to submit a proposal will receive further guidance in time to meet a December 1, 2008, application deadline. Applicants are encouraged to contact Shirley Hamilton, Assistant Director of Programs, before submitting anything in writing. The Letter of Intent must be signed by the primary contact for the request, as well as by an officer or top staff (executive director, president, department chair, dean, etc.) of the requesting organization.

 

Second Step is the Grant Request
The deadline for submitting a Grant Request is December 1, 2008
If your Letter of Intent passes the initial review stage, we will invite you to submit a grant request for your project, following the instructions below. Be sure to address any concerns the review committee may express in the letter of notification that you will receive.

 

Instructions for Internal Review, New Grant Requests
If you are invited to submit a Grant Request based on your Letter of Intent, your grant request should include the following information, printed in 12 pt. typefont with 1” margins, without binders, notebooks, folders or staples. Use paper clips or binder clips only. Note: We photocopy all submitted material, for distribution to the review committee. Keep this in mind if any part of your request is in color.

Submission Methods:
• Submit 1 signed hard copy of the grant request, including the supporting documentation, to the ESFA office.
• Your grant request, in its entirety, must also be submitted via email to be considered for funding. The application must be provided in Microsoft Word or a similar editable format (not PDF). Information Required:

1. Organization Information - organization name (for UAB requests, also list school and/or department); organization’s primary contact (i.e., President, CEO, dean, department chairman) name, title and address.

2. Applicant Information - primary contact name/title (this is the person most familiar with the grant application, whom we will contact about the grant. This is often someone other than the Organization’s primary contact); address/city/state/zip; phone; fax; email; web address. For UAB Applicants: provide campus mail address and campus zip code. The application must be signed by the primary contact for the request, as well as by an officer or top staff (executive director, president, department chair, dean, etc.) of the requesting organization.

3. Project Information - Project Information - project title; project summary (one-paragraph description); total project cost; amount of request; one, two or three-year grant request (include breakdown of amount requested for each year).

4. Organization Background - mission; history; major programs; accomplishments; links with similar organizations.

5. Project or Program - specific eye care need or issue you will address; goals and objectives; activities that will be carried out to accomplish the objectives; key personnel and their qualifications; other organizations involved in your project or providing similar services.

6. Project Continuation - if the project is ongoing, your plans to continue after the funding period; future funding sources.

7. Project Evaluation - your criteria for effectiveness; the methods you will use to analyze your results (measure your progress); who will assess the results?

8. Additional Funding - A list of other foundations or sources to which you have submitted this proposal.

9. Results - Indicate whether funds have been committed, declined or are pending from those submitted proposals.

10. References - Names, titles and telephone numbers of three professionals familiar with the work of your organization.

11. Budget - A line item income and expense budget for the project, including in-kind and cash support. The budget may include costs for equipment, personnel, supplies, and consultants. It may not include indirect costs or simple replacement of existing salary support.


Supporting Documentation: non-UAB applicants must provide the following:
12. Your organization's current annual operating budget and an income and expense statement for your last fiscal year. Both should include sources of support as well as expenses.

13. A tax return or audited financial statement.

14. A list of your current Board of Directors, including the primary professional and/or civic affiliation of each member.

15. Your organization's tax-exempt letter from the IRS.
